Position Summary

Responsible for coordination of care with Primary Care Physicians, Pediatricians, Psychiatrists and other medical or mental health treatment providers and performing whole-person assessments; make recommendations to the internal Care Teams on interventions and provide education and awareness for preventive care and health promotion.  Promote smoking prevention and cessation, substance abuse prevention and cessation, nutritional counseling, obesity reduction, and increased physical activity. Engage member and/or caretaker on alternatives to emergency or hospital care. Participate in hospital discharge process. Perform medication reconciliation and aid with medication, treatment management, and adherence, including administering oral and IM medications as needed.

Requirements

• RN or BSN License in Iowa
• Valid Driver’s License and Insurance

Preferred Qualifications

Bilingual a plus, but not required
