ACT Case Manager/Employment Specialist

PAMHC is seeking a full-time Case Manager/Employment Specialist for our Assertive Community Treatment (ACT) team based at our Carroll Office to cover several counties in Northwest Iowa. Travel required for in home and community-based services for clients. Position is an average of 40 hours per week Monday – Friday, with periodic on-call required.

Pay Range:

$23.77 starting wage

Position Summary:

This position primarily involves weekly coordination of care for ACT clientele with a multifaceted team of providers to ensure courteous, helpful, culturally competent and respectful services. Additional duties as follows: utilization of skills gained through vocational rehabilitation training to provide employment support as needed. Work location and population may vary depending on organizational needs.

Requirements:

• Valid Driver’s License and Insurance

• Bachelor’s degree in human services field, with one-year experience working with persons with serious mental illness

• Licensed RN in the state of Iowa with at least three years’ experience working with persons with serious mental illness

Preferred Qualifications:

Bilingual a plus, but not required
